The First 24-72 Hours After Surgery: Your Most Crucial Window

The first few days following a FUE or FUT hair transplant are essential for protecting the newly implanted grafts and minimising discomfort.

Sleep with your head elevated at a 45-degree angle using 2 to 4 pillows. This position helps to reduce swelling and avoid pressure on the grafted area. It’s also wise to place an old towel over your pillow to catch any potential minor bleeding. To manage swelling, apply an ice pack or a bag of frozen peas below the grafted area (never directly on it). Use for 10-15 minutes every 4-6 hours, starting the day after your surgery. Always wrap the ice pack in a clean towel.

Use the pain relief medication provided by your surgical team, and follow all dosage instructions. Avoid alcohol, operating machinery, or driving while on pain medication. If you’re unsure about what you can safely take, contact our team.

Medication, Movement & What to Avoid

For the first 48 hours, do not use aspirin or any aspirin-containing products. Alcohol should be avoided for at least 24 hours. For 2 days, avoid bending down to the floor – get down on your knees instead to pick something up.

It’s also essential to pause all strenuous activity, including going to the gym, running, or even brisk stair climbing, for at least 7 days as increased heart rate and blood pressure may exaggerate swelling and bleeding. Sweating can interfere with healing and increase the risk of infection or graft disturbance.

Hair Washing Routine: Day-by-Day Instructions

Days 1-4: Gently Cleanse With Care

Do not wash your hair during the first 24 hours post-procedure. Starting on day 2, you can begin washing your scalp gently.

If using a shower, keep the pressure low. Place the clean towel provided over the grafted area and allow warm water to soak through. Gently pat the towel with your hand. Apply the shampoo to the wet towel and pat the area for around 30 seconds. Rinse using the same method, ensuring all soap is removed to avoid irritation. Never rub the area – only ever dab gently to dry.

This method can also be done with a large cup if no shower is available.

Days 5-7: Continue the Same Process

Continue using the towel method, gently rinsing and patting your scalp daily. By this stage, some scabbing may occur – this is normal. Do not try to remove scabs early, even if hair comes out with them. The roots remain intact and your new hair will begin growing in 12–16 weeks.

After 7 Days: Transition Back to Routine

From day 7 onwards, you can begin washing your hair more normally – without the towel. Start incorporating the conditioner if your hair is left long, applying it gently to your scalp. Leave it on for one minute before rinsing. You may now use a hair dryer, but only at a safe distance (at least 30 cm from your scalp), and on a low heat setting.

You can also use a comb gently but avoid hairbrushes. Avoid using Minoxidil or any styling products on the grafted area for at least 7 days following your procedure.

What Not to Do After a Hair Transplant

Understanding what to avoid is critical for protecting your results. Here are some of the most common mistakes to steer clear of:

  • Scratching or rubbing the grafted area
  • Using hair products or Minoxidil in the first 7 days
  • Participating in physical activity too soon
  • Wearing tight hats or helmets before advised
  • Removing scabs before they naturally fall off
  • Drinking alcohol or using aspirin in the first 48 hours

FAQ: Common Hair Transplant Aftercare Questions

Q: When can I wash my hair after transplant?

You can begin gently washing your hair 24 hours after surgery using the towel method as directed above.

Q: Is itching normal after a hair transplant?

Yes, mild itching is common during the healing phase, but please do not scratch the area as this can compromise the end results.

Q: Can I wear a hat after hair transplant surgery?

Yes, as long as they are loose fitting and not put pressure on the implanted grafts.

Q: How long until the grafts are secure?

By day 6 to 7, your grafts are generally secure and unlikely to be dislodged by routine care, but we still advise you’re gentle with your hair for at least the first month.

Q: When will new hair start to grow?

Hair starts to grow approximately 12–16 weeks post-procedure, with visible results improving month by month. You’ll be booked in for a follow up appointment at 8 months at the latest so we can assess your results and discuss ongoing management.
